Building a Cancer Screening and Prevention Program
Developed a community-driven colorectal cancer screening initiative in partnership with community stakeholders, securing over $1 million in funding, reaching more than 10,000 people, and connecting 100 community members with screening — directly addressing a high-priority disparity in an underserved population.
Creating a Library of Health Literate
Resources
Developed a library of over 100 educational guides and health literate resources to support shared decision making and patient engagement among individuals living with chronic conditions, such as diabetes, cardiovascular disease, and obesity.
Improving Vaccination Rates in Physician Practices
Partnered with a multi-site clinical network to improve vaccination rates, achieving measurable improvements through physician partnership and strategies to improve ease of recommendation.
